A military explosive plant in Tennessee exploded on Monday, local time. State officials said on 11th that no survivors were found at the explosion site, and that all 16 missing people could be estimated to have died.

The factory had an explosion in 2014, killing one person and injuring many others. In 2019, the factory was again fined by the Tennessee Occupational Safety and Health Administration for reasons including improper storage of hazardous chemicals, insufficient employee training, and lack of monitoring of worker health risks.
